These Terms govern your use of the Trellis Vitality platform, including the website, member portal, and clinical telehealth services facilitated by Trellis Vitality, LLC. By creating an account or completing the Free Vitality Assessment, you agree to these Terms.

Clinical scope

Trellis is a technology platform that facilitates access to independent, licensed clinicians and US pharmacies. Trellis does not provide medical advice, diagnosis, or treatment. Prescriptions are issued at the sole discretion of independent providers based on the clinical appropriateness of each individual case.

Eligibility & state availability

You must be 18 years or older and reside in a state where the licensed clinician network is active. Some protocols carry additional clinical criteria.

Services are available in all 50 US states through affiliated medical providers and pharmacy partners operating in accordance with applicable state licensure requirements. Please note that certain medications, formulations, or fulfillment options may vary depending on state-specific pharmacy regulations and dispensing restrictions.

Payment and refunds

Prescription and protocol fees are described at checkout. There is no membership requirement; plans differ only by duration and you may pause or cancel anytime. Refund eligibility is determined on a per-order basis — contact customer support for assistance.

Limitation of liability

To the maximum extent permitted by law, Trellis Vitality, LLC is not liable for indirect, incidental, or consequential damages arising from use of the platform or services facilitated through it.

Disputes

Disputes are resolved through binding arbitration under the rules of the American Arbitration Association, unless prohibited by your state of residence.
